
| Human Resources Question of the Week: Do I need an ID card to use the university's new vision provider?Yes, an ID card is required to use EyeMed, the nation’s largest vision plan provider. Kent State faculty and staff who selected medical coverage with Medical Mutual SuperMed Plus PPO or Anthem Blue Cross and Blue Shield for 2010 are automatically enrolled in EyeMed, and should have received ID cards that were mailed to their homes. If you did not receive your ID card, please contact EyeMed Customer Service at 1-866-299-1358. The EyeMed plan provides access to a large group of independent practitioners and optical retail providers, including LensCrafters, Pearle Vision, Sears Optical and Target. When using an in-network provider, employees do not have to pay in advance and file a reimbursement claim. If you choose an out-of-network provider for services, you will be required to pay all costs up front and submit a reimbursement form to EyeMed.
