Who is the university's vision care vendor?

In January of this year, EyeMed became the vision provider for Kent State University. EyeMed is the nation's largest vision plan provider and gives members access to a large group of independent practitioners and optical retail providers, including LensCrafters, Pearle Vision, Sears Optical and Target. When you elect medical coverage with Medical Mutual SuperMed Plus PPO or Anthem Blue Cross and Blue Shield, EyeMed is automatically selected. The plan is on a rolling 24-month calendar.

Any employee with Kent State medical coverage should havereceived an ID card earlier this year for the EyeMed vision program. EyeMed does not have a separate membership identification number; however our group number is 9765785.

You may receive service from any vision care provider. If you use an in-network provider you do not need to submit a claim form. To see a list of participating providers near you, go to www.enrollwitheyemed.com and choose "SELECT" from the provider locator dropdown box. You can also call 1-866-299-1358. If you are using an out-of-network provider you must submit a claim form to EyeMed for reimbursement of covered expenses. The EyeMed vision claim form is available in the Human Resources Forms Library.
